Leaving the hospital or clinic comes with a stack of instructions — medications at different times, wound care, follow-up appointments, activity limits. dischargeRx exists to make that stack easy to actually follow. And because care rarely stops when the recovery does, it keeps going for the conditions you manage every day after.

What we do

dischargeRx imports your discharge instructions and turns them into one clear, organized schedule with perfectly timed reminders for medications, treatments, tasks, and appointments. You can follow it yourself, or invite a trusted family member to help — with only the access you choose to grant.

For ongoing conditions like high blood pressure, type 2 diabetes, or a thyroid medication, dischargeRx builds an open-ended Maintenance Plan instead of a countdown. Every condition you add folds into that same plan, progress shows up as a day-by-day streak rather than a finish line, and when a recovery wraps up the medications you'll keep taking can move straight across.

• Your data is yours. It stays on your device by default, we don't sell it, and you can export or delete it anytime. See our approach to security.
